New to the Austin Area and don’t know where to start with your Orthodontic transfer case? Ortho 360 is here to help! All you have to do is contact us to schedule a complimentary consultation with us.
How to Transfer?
1. Reach out to one of our branches, to get your complimentary consultation scheduled. We have 6 locations in Austin and surrounding areas.
2. Request your previous Orthodontic provider to complete the AAO form. Once the form is completed, make sure they email the form and your records over to us at info@ortho360.com
3. The AAO form is needed prior to your orthodontic consultation.
4. During your consultation we will review the continuation of your orthodontic treatment plan.
